I would check to see if you have a worped head or block. Or maybe you have some coolent area blocked in the motor and it is causing an over heating issue. Or it could not be torqed properly. I would tear the motor down and check everything completely. Check you entire coolant system. Have your head re-surface and groud down. Have your block checked for cracks or deformities.

Now, you said the head looks okay, but was it ever re-surfaced, or checked with a percision machined straight edge? How about the block? This is very important. Also, was the proper torguing sequence performed...and was it later re-torqued? Just want to rule these items out.
