Tob.... Again!

wtf did i do wrong while replacing a throw out bearing that it is out AGAIN!??! this brand new OEM throw out bearing has less than 5000 miles on it, and again, i am getting a scream when i push the clutch in when the car is cold. exactly how it started last time, except it started happening when the car was warmer and warmer. i don't see what i could have done wrong with the installation for it to be happening again. all i can think of to do is to replace it again, and do the whole clutch this time. it's hard to tell how long it's been since that's been done. but seriously, WTF is wrong with my car that it is going through throw out bearings like this?

Do you hold the clutch in a lot? (Like at stop lights and the like.)
